Complete the Princess Royal University Hospital antenatal self-referral form to refer yourself and email it to kch-tr.br-maternitypruh@nhs.net. You do not have to see your GP before contacting us. We will email you to confirm we have received your form.

Referrals from GPs

GPs can also refer to all clinics using the Antenatal GP referral form. Email it to kch-tr.br-maternitypruh@nhs.net. Bromley GPs should use the Referrals Optimisation Tool to refer patients, however self-referral should be encouraged.

For community bookings where a nuchal scan is required, please submit the form and state which community midwifery team is providing care.
