Complete the online antenatal self-referral form to refer yourself to Princess Royal University Hospital. You do not have to see your GP before contacting us.

If you have a MyChart account with us, please use the ‘Sign-in’ option on the form to complete the referral. If you do not have a MyChart account you can fill in the form without one, and create your account in the process.

Please fill out as much of the form as possible and provide your correct contact details.

Contact the Maternity team if you cannot submit the form, or if you do not receive your first appointment details within 7 working days of submitting the form. You can contact the team by emailing kch-tr.br-maternitypruh@nhs.net or calling 020 3910 7279.

The information you provide will be placed in your medical records and only accessed by staff involved in your care. Please be aware we liaise with relevant partner agencies as required. Read more about your rights in our Privacy Notice.

Referrals from GPs

GPs can also refer to all clinics using the Antenatal GP referral form – PRUH. Email it to kch-tr.br-maternitypruh@nhs.net. Bromley GPs should use the Referrals Optimisation Tool to refer patients, however self-referral should be encouraged.

For community bookings where a nuchal scan is required, please submit the form and state which community midwifery team is providing care.
